Description:
The login already has an account under a different user name.
Severity 16 Description:
Indicates general errors that can be corrected by the user.
Identifying SQL Server Error Log File used by SQL Server Database Engine can be done by reading SQL Server Error Logs. DBA can execute the XP_READERRORLOG extended stored procedure to read the SQL Server Error Log and search for its location used by the instance of SQL Server.

Easy fix for this error would be
The login was incorrectly linked to the database as the dbo. Because this account built a database during the installation, this happened.
sp_changedbowner 'sa'
Then just re-map your login DomainName\UserName to the database (double click on the login->User Mapping->check your DB->check Datareader/Datawriter->Ok)
